IMAGE CLAUSE

I consent to the free recording of my image in the form of a film and photo by Oxygen Clinic LTD The consent covers the use, recording and reproduction of the photos taken via any media for any purpose compatible with applicable law and regarding the service of the Normobaric Chamber, including publication in social media channels, website, folders, electronic publications, websites, etc.
At the same time, I renounce the rights related to the control and approval of each use of the film and photography with my image.
I declare that the use of the image in accordance with this Consent does not violate anyone’s personal rights or other rights.
I declare that I have read the content of the above consent in full and that I fully understand it.

STATEMENT

1. I have read and understood the Terms and Conditions of the session services in a Normobaric Chamber made available to me and I accept its content and undertake to comply with its terms (in particular with regard to the Service Recipient’s obligations).
2. I agree to comply with the requirements of the building located at Westmill Street, Stoke On Trent ST1 3EL – Regulations and instructions issued to me by employees or representatives of the Oxygen Clinic
3. There are no contraindications for my use of the normobaric chamber
4. I am fully aware that normobaric exposure is not a medical procedure
5. I use the Normobaric chamber at my own risk and I am aware of the consequences and dangers of non-compliance with the Regulations, non-compliance with the recommendations of the Facility staff, or concealing my actual health condition.

Terms and Conditions

1. Before entering the chamber, the user gets acquainted with the contraindications to the use of normobaric oxygen therapy, i.e. acute sinusitis, acute otitis, optic neuritis, undiluted pneumothorax, pregnancy, the period immediately after cataract surgery, , , state after drinking alcohol, intoxicants and medications that may cause disturbance of consciousness
2. A person who wants to use the normobaric chamber, after getting acquainted with the contraindications, and accepting them, enters at their own risk.
3. People with an infection of the upper respiratory tract, for reasons of hygiene, are asked to postpone the visit to the normobaric chamber.
4. Before entering the chamber, please notify the operating staff about your health, possible pregnancy, slow heart rate and individual coexisting diseases.
5. Please respect the peace and quiet during the normobaric session.
6. The day before the planned session in the normobaric chamber, you will receive a text message reminding you about the visit. Please confirm or cancel your visit no later than 24 hours before the scheduled session date. If the visit is not cancelled, the session will be charged against the subscription.
7. Payment for the use of the Normobaric Chamber is made before entering the chamber on our website or in the reception
8. All the outerwear (jackets/coats) are left in the wardrobe in the reception area. Please do not leave in any valuables (e.g. bags, wallets, phones, keys). The staff is not responsible for items left unattended.
9. It is forbidden to touch the apparatus and devices inside the chamber under penalty of £500
10. It is forbidden to use open fire, sparks and other activities that may cause ignition,(including cigarettes and electronic cigarettes) subject to a fine of £100,000.
11. Contact with staff is via intercom. The available cameras are: in the Standard room on the table, in the VIP room on the wall by the door, in the lock on the wall by the door to the chamber. Please dial number 1 to the reception desk to make contact.

12. The customer bears full financial responsibility for any damage caused by the customer’s actions not complying with the regulations.
13 There is a strict ban on smoking tobacco products, electronic cigarettes, drinking alcohol and eating meals in the chamber and at the reception desk.
14. Children under 16 years of age benefit from sessions under the supervision of a parent or guardian. Guardians are obliged to look after their children. All damage done by children is covered financially by the legal guardians.
15. Entry to and exit from the normobaric chamber takes place with the direct participation or assistance of staff. Entries take place every 2 hours:
Monday – Friday: 9:30-11:30 | 11:30-13:30 | 13:30-15:30 | 15:30-17:30 | 17:30-19:30 | 19:30-21:30

Saturday: 09:30-11:30 | 11:30-13:30 | 13:30-15:30 15:30-17:30 | 17:30-19:30

16. The client is obliged to appear 20 minutes before the scheduled time of the session in the chamber. Late delay results in the cancellation of the reservation.
17. In the event of a need to suddenly leave the normobaric chamber, notify the staff via the intercom about the situation or, as a last resort, press the red emergency button on the wall in the main room.
The chamber becomes unsealed within 4 minutes and automatic door opening. Attention! Unjustified use of the emergency button is associated with additional costs of refilling the chamber in the amount of £7,500
18. The personnel may refuse the entry to the Normobaric chamber if they have grounds for doing so (e.g. confirmed infection, condition indicating alcohol consumption).
19. Monitoring is installed in the chamber and at the reception desk. The purpose of monitoring is to protect against activities that may cause damage to the workplace, customer property and to protect administrative documentation. The information recorded will not be used for purposes other than those listed above. It is noted that the monitoring will interfere with the privacy of customers as little as possible.
20. The minimum session time is 2hours
21. The personnel may require putting on a disposable protective mask, disposable protective shoes and disinfecting hands at the entrance to the Facility.
22. Using our services is tantamount to accepting these regulations and complying with the applicable rules.
23.The Relax section can accommodate up to one wheelchairs per session, this need to be arranged in advance by calling 01782901132/07741573047, the VIP section can not accommodate any wheelchairs
24. rescheduling is available up to 3 days before the appointment, subject to availability
